About Hayat Pharmacy

The mission of Hayat Pharmacy is to empower people to live their best lives through quality, compassionate, solution-driven healthcare. Founded in 2011 by Hashim Zaibak, PharmD, Hayat is independently owned and offers exceptional care at 19 community pharmacies throughout Southeastern Wisconsin and through its Long-Term Care Pharmacy that serves the needs of communities across Wisconsin. Our services include FREE prescription delivery and specialized medication packaging, in-depth pharmacist consultations, vaccinations, safe medication disposal, and durable medical equipment. Hayat Pharmacy is part of the Health Mart franchise and was named the Health Mart Pharmacy of the Year in 2014 and 2021.
