Vaccine Clinics
For adults and children ages 3 and older, schedule a flu shot, COVID-19 vaccine or booster, or other vaccine at our Vaccine Clinic (807 W. Layton Ave., Milwaukee) using the calendar shown below. Walk-in service is also available!
To receive a vaccine at your nearest Hayat, call the location to schedule an appointment in advance over the phone.
Vaccines for children ages 6 months to 2 years old are available at our Vaccine Clinic only and appointments are required in advance.
Please print a completed Vaccine Consent Form and bring it to your appointment, along with your insurance card, if available.
